En 1967, alors que j’étais adolescent (15 ans), j’ai été traité pour un sarcome embryonnaire du visage (en arrière de la joue gauche) avec une chirurgie limitée de la face, suivie de chimiothérapie intensive puis de curiethérapie. Cette dernière a consisté en l’insertion d’une source d’iridium (matériel radioactif local) dans des gaines en plastique qui ont traversé ma joue pendant 8 jours, et qui a provoqué une brûlure immédiate des tissus de cette zone avec une suppuration importante qui a gêné mon alimentation, mais aussi la formation de croûtes épaisses externes sur la peau. Les effets de cette brûlure ont disparu peu à peu par des soins locaux, mais des effets à plus long terme ont commencé à se manifester malgré la satisfaction de la guérison obtenue !

Ces effets secondaires progressifs ont été d’ordre esthétique, avec l’apparition de marbrures sur une peau de plus en plus dure et fragile et d’une déformation progressive de l’os de la mâchoire situé en-dessous, dont la croissance n’était pas achevée. Les effets ont été également fonctionnels, avec l’apparition de brides épaisses tendues à l’intérieur de la joue gênant l’ouverture de la bouche (et tirant la lèvre sur le côté), tandis que les muscles de la joue fondaient, se transformant progressivement en une sorte de peau de tambour sans souplesse qui m’empêchait d’ouvrir la bouche et de pratiquer une parfaite hygiène dentaire.

Ainsi les soins dentaires ont été rendus difficiles, avec en quelques années une dégradation de la gencive, devenant inquiétante lorsque de l’os a été mis à nu et que mes dents ont commencé à bouger. L’état de ma joue n’était pas trop bon non plus, car outre le « sourire » modifié pendant plusieurs années, j’ai présenté une inflammation de la peau suivie d’une fistulisation (communication entre l’extérieur et l’intérieur de la joue par un trou) qui a été soignée à l’époque par de simples traitements antibiotiques successifs, mais qui n’auguraient rien de bon.

En 2013, alors âgé de 61 ans (et 46 ans après cette radiothérapie), lors d’une conférence sur les effets à long terme des traitements contre le cancer, j’ai rencontré le trésorier de l’association des Aguerris (association dédiée au suivi des cancers de l’enfant), patient lui-même porteur de séquelles liées à une radiothérapie de l’enfance. Il m’a indiqué le travail du Dr Delanian qui le suivait à ce sujet.

Lors de la consultation, le Dr Delanian m’a été proposé un traitement antifibrosant PENTOCLO, que j’ai suivi de 2013 à 2017, et qui a eu un effet spectaculaire sur ma santé : en 3 ans, l’os de ma mâchoire supérieure (maxillaire) s’est redensifié, l’état de ma gencive s’est amélioré avec des dents moins mobiles, et les brides à l’intérieur de ma joue ont disparu.

Il était temps, car juste avant ce traitement PENTOCLO, j’avais dû me faire arracher deux dents qui bougeaient à l’IGR. Ces extractions dentaires ont été complétées par une chirurgie de couverture par lambeau muqueux afin de prévenir une communication bucco sinusienne, source potentielle de grosse complication infectieuse.

Enfin début 2020, lors d’un examen de routine, ma dermatologue de ville a remarqué une lésion cutanée évoquant un carcinome basocellulaire radio-induit sur ma joue gauche, et a demandé une biopsie pour en avoir le cœur net. Bien heureusement, cette biopsie de joue s’est révélée négative (pas de cancer), mais a provoqué une plaie avec retard de cicatrisation de ma joue (fistule qui ne se refermait plus).

Le Professeur DELANIAN consultée alors à ce sujet, m’a remis sous traitement PENTOCLO pour activer la cicatrisation. Au bout d’un an environ, après amélioration déjà bien nette, elle m’a recommandé de compléter ce résultat avec un lipofilling car, même cicatrisé, le déficit de tissu était très important. Le lipofilling (injection de graisse prélevée au niveau de la peau du ventre, donc de cellules fraiches non irradiées, puis après centrifugation, réinjectée là où cela peut être nécessaire) a été réalisé par une chirurgienne maxillo-faciale de l’IGR en janvier 2021. Un peu plus d’un mois après, le résultat était presque parfait. Une deuxième injection de lipofilling devrait consolider la première car les cellules injectées delà première injection se fondent souvent dans les tissus qui manquent encore d’épaisseur. La plaie fistulisée est aujourd’hui complètement refermée, ma joue semble avoir retrouvé un état local bien meilleur et je suis très satisfait.

In 1967, when I was a teenager (15 years old), I was treated for embryonal sarcoma of the face (back of the left cheek) with limited facial surgery, followed by intensive chemotherapy and then brachytherapy. The latter consisted of the insertion of a source of iridium (local radioactive material) in plastic sheaths which passed through my cheek for 8 days, and which caused an immediate burn of the tissues of this area with significant suppuration which interfered with my diet, but also the formation of external thick crusts on the skin. The effects of this burn gradually disappeared with local care, but longer-term effects began to appear despite the satisfaction of the healing obtained!

These progressive side effects were aesthetic in nature, with the appearance of mottling on an increasingly hard and fragile skin and a progressive deformation of the jaw bone below, the growth of which does not was not completed. The effects were also functional, with the appearance of thick flanges stretched inside the cheek hindering the opening of the mouth (and pulling the lip to the side), while the muscles of the cheek melted, transforming gradually into a sort of limp drumhead that prevented me from opening my mouth and practicing perfect dental hygiene.

Thus dental care was made difficult, with a deterioration of the gums in a few years, becoming worrying when the bone was exposed and my teeth started to move. The state of my cheek was not too good either, because in addition to the « smile » modified for several years, I presented an inflammation of the skin followed by a fistulization (communication between the outside and the inside of the cheek through a hole) which was cured at the time by simple successive antibiotic treatments, but which did not bode well.

In 2013, then aged 61 (and 46 years after this radiotherapy), during a conference on the long-term effects of cancer treatments, I met the treasurer of the Association des Aguerris (association dedicated to follow-up of childhood cancers), patient himself carrying sequelae related to childhood radiotherapy. He pointed me to the work of Dr. Delanian who was following him on this subject.

During the consultation, Dr. Delanian was offered a PENTOCLO antifibrosis treatment, which I followed from 2013 to 2017, and which had a spectacular effect on my health: in 3 years, the bone of my upper jaw (maxilla) has become denser, the condition of my gums has improved with less mobile teeth, and the flanges inside my cheek have disappeared.

It was time, because just before this PENTOCLO treatment, I had to pull out two teeth that were moving at the IGR. These dental extractions were completed by mucosal flap cover surgery to prevent oral sinus communication, a potential source of serious infectious complications.

Finally at the beginning of 2020, during a routine examination, my city dermatologist noticed a skin lesion evoking radiation-induced basal cell carcinoma on my left cheek, and asked for a biopsy to find out for sure. Fortunately, this cheek biopsy turned out to be negative (no cancer), but caused a wound with delayed healing on my cheek (fistula that no longer closed).

Professor DELANIAN then consulted on this subject, put me back on PENTOCLO treatment to activate healing. After about a year, after an already clear improvement, she recommended that I complete this result with lipofilling because, even if I had healed, the tissue deficit was very significant. Lipofilling (injection of fat taken from the skin of the belly, therefore of fresh non-irradiated cells, then after centrifugation, reinjected where necessary) was carried out by a maxillofacial surgeon from the IGR in January 2021 A little over a month later, the result was almost perfect. A second injection of lipofilling should consolidate the first because the cells injected after the first injection often blend into the tissues that still lack thickness. The fistulized wound is now completely closed, my cheek seems to have regained a much better local state and I am very satisfied.
