[:fr]En 2003, j’ai été opérée d’un cancer du sein droit, traité ensuite par chimiothérapie et radiothérapie, en particulier au niveau ganglionnaire axillaire (sous le bras) et sus-claviculaire (au-dessus de la clavicule) et bien contente de m’être débarrassée du cancer !
Quinze ans après, en 2017, j’ai commencé à avoir des fourmillements dans l’auriculaire et l’annulaire de la main droite avec un début d’engourdissement. Je souffrais également du côté droit, depuis quelques années (2008), de migraines, de douleurs à l’œil et à l’oreille droits pulsatiles, et de vertiges. Tous les médecins consultés pour cela au fil du temps ne trouvaient rien de spécial… L’IRM du cerveau était normale, au point de laisser entendre que je m’imaginais des soucis du côté du cancer.
Suite à la garde prolongée de mon petit-fils pendant deux semaines, un beau bébé de 9 mois réclamant les bras à longueur de journée, l’état de mon bras s’est aggravé par étirement des nerfs. Après consultation d’un neurologue, le diagnostic était posé : séquelle tardive de la radiothérapie au niveau du plexus et nerfs du bras droit. Ce médecin m’a informé alors qu’il n’y avait rien à faire et que cela allait s’aggraver à petite ou grande vitesse, on verrait bien ! Une sorte de condamnation sans appel.
C’est alors que j’ai pris conscience que je ne voulais pas en rester là, et j’ai commencé des recherches sur Internet. J’ai trouvé un article du Pr Sylvie DELANIAN qui répondait à mes questions sur la plexite, enfin ! Je me suis surprise à danser de joie dans mon salon. Avais-je trouvé la perle rare ?
Le premier rendez-vous de consultation dédié a été une révélation. Enfin un médecin qui écoute et comprend avec, sans nul doute, des compétences incroyables. Je remercie le Pr Sylvie DELANIAN d’exister. Elle a confirmé le diagnostic de séquelle tardive de radiothérapie la plexopathie radio-induite du plexus brachial droit, en s’étonnant de la non poursuite des investigations. J’ai bénéficié dès le premier jour d’un traitement anti-inflammatoire et fluidifiant pour un mois, le temps de finir les explorations, et déjà la douleur au poignet avait disparu.
Une imagerie IRM du plexus nerveux associée à des séquences angiographiques a permis ensuite de rechercher puis découvrir une sténose à 95 % (rétrécissement serré) de l’artère sous-clavière droite prévertébrale en relation avec la radiothérapie au niveau de la base du cou. !Une exploration par artériographie suivie de pose de stent dans l’artère sousclavière droite presque bouchée a été réalisée en urgence Ce geste m’a sauvé la vie, car il y avait une menace très importante d’accident vasculaire cérébral (AVC) et perte de l’usage de mon bras. Depuis, comme par magie, les problèmes de la tête du côté droit ont disparu après 10 ans de gêne : mon cerveau et mon bras sont bien vascularisés.
Puis en relais a été mis en place le protocole antifibrosant « PENTOCLO » pour traiter la plexite. Ces médicaments me fatiguent un peu mais cela n’est rien en comparaison du bénéfice et de l’espoir que je mets dans ce traitement. Je dois être à l’écoute de mon bras et ne pas faire d’excès comme par exemple deux heures de repassage ou de ménage intensif. J’apprends tous les jours à ménager mon bras et ma main, mais les utilise pour ne pas les affaiblir. Dès que des douleurs apparaissent dans la main et comme j’ai de moins en moins de force de préhension, je trouve des objets qui m’aident, notamment dans ma cuisine : un crochet spécial pour ouvrir sans effort les boîtes de conserve avec anneau, un ouvre bocal automatique !
J’ai appris ainsi qu’un mal peut en cacher un autre, les complications dans le volume de radiothérapie peuvent être multiples, certaines à menace immédiate, d’autres lentes et chroniques. J’ai appris également que ces séquelles ne sont pas qu’une fatalité. Bien que je ne sois qu’au début de mon traitement, je reste POSITIVE car j’ai une confiance totale envers le Pr Sylvie DELANIAN.
Danielle V.[:en]In 2003, I underwent surgery for right breast cancer, then was treated with chemotherapy and radiotherapy, in particular in the axillary (under the arm) and supraclavicular (above the clavicle) lymph node level and was very happy to get rid of the cancer!
Fourteen years later, in 2017, I started to have tingling in the little and ring fingers of the right hand with the beginning of numbness. I also suffered on the right side, for a few years (2008), migraines, pain in the right eye and ear pulsatile, and dizziness. No doctors consulted for this over time could find anything special … The MRI of the brain was normal; to the point of suggesting that, I imagined, cancer concerns.
Following the extended custody of my grandson for two weeks, a beautiful 9 month old baby claiming my arms all day long, the condition of my arm worsened by stretching my nerves. After consultation with a neurologist, the diagnosis was made: latent after-effects of radiotherapy at the level of the plexus and nerves of the right arm. This doctor informed me that there was nothing to do and that it would get worse at low or high speed, we would see! A sort of condemnation without appeal.
It was then that I realized that I did not want to stop there, and I started researching the Internet. I found an article by Pr Sylvie DELANIAN, which answered my questions on plexitis, finally! I found myself dancing with joy in my living room. Did I find the rare pearl?
The first dedicated consultation meeting was a revelation. Finally a doctor who listens and understands with, without a doubt, incredible skills. I thank Professor Sylvie DELANIAN for existing. She confirmed the diagnosis of latent radiotherapy after-effects, radio-induced plexopathy of the right brachial plexus, and was surprised by the non-pursuit of investigations. I benefited straightaway from anti-inflammatory and thinning treatment for a month, the time to finish the explorations, and already the wrist pain was gone.
MRI imaging of the nervous plexus associated with angiographic sequences then made it possible to search for and discover a 95% stenosis (tight narrowing) of the right pre-vertebral subclavian artery in connection with radiotherapy at the base of the neck! An exploration by arteriography followed by placement of a stent in the almost blocked right subclavian artery was carried out as a matter of urgency. This gesture saved my life, because there was a very significant threat of a cerebrovascular accident (stroke) and loss of using my arm. Since then, as if by magic, the problems of the head on the right side disappeared after 10 years of embarrassment: my brain and my arm are well vascularized.
Following that, I was prescribed the anti-fibrosis protocol « PENTOCLO » to treat plexitis. These drugs tire me a little but that is nothing compared to the benefit and the hope that I put in this treatment. I have to listen to my arm and not overdo it, such as doing two hours of ironing or intensive cleaning. I learn to spare my arm and my hand every day, but use them so as not to weaken them. As soon as pain appears in the hand and as I have less and less grip strength, I find objects that help me, especially in my kitchen: a special hook to effortlessly open cans with a ring, an automatic jar opener!
I learned that one disease can hide another, complications in the volume of radiotherapy can be multiple, some of immediate threat, others slow and chronic. I also learned that these consequences are not just a fatality. Although I am only at the beginning of my treatment, I remain POSITIVE because I have total confidence in Pr Sylvie DELANIAN.
