[:fr]Membre de l’ARSER, j’ai témoigné lors de l’émission « Allo Docteurs » le lundi 23 mai 2016 sur France 5, en tant que patient présentant des séquelles tardives d’une radiothérapie ORL.
Le bureau de notre Association a jugé utile que j’expose, dans un éditorial, mon parcours qui m’a conduit à une guérison et une vie acceptable. Alors âgé de 48 ans, j’ai présenté un cancer ORL en 1994. Des odeurs buccales inhabituelles étaient apparues : j’ai consulté mon stomatologiste habituel lequel, après examen, m’a confirmé que la cause n’était pas dentaire. Une gêne à la déglutition était également présente, mon oto-rhino a constaté une anomalie à la base de langue. Après biopsie, il m’a appris que je présentais une tumeur cancéreuse (carcinome peu différencié infiltrant ulcéré).
J’ai donc été dirigé vers une unité spécialisée ORL à l’Hôpital Bichat (Dr Joël Depondt). A l’issue de cette consultation, une opération chirurgicale de la langue semblait évidente malgré les conséquences inévitables sur le plan esthétique et les difficultés ultérieures pour la parole et l’alimentation. Inutile de vous dire que j’étais SOUS LE CHOC.
Suite à un nouveau rendez-vous avec le Dr Depondt, celui-ci m’informe que lors d’une réunion entre médecins, dont le Dr Sylvie Delanian Oncologue Radiothérapeute, il a été proposé un traitement par RADIOTHERAPIE EXCLUSIVE au Cobalt (à la dose 45Gy) associée à une CHIMIOTHERAPIE (Carboplatine) suivi de CURITHERAPIE à l’Iridium à la dose 30 Gy (Dr Delanian Hôpital St-Louis).
Six semaines après ce traitement conservateur, la tumeur avait complétement disparu.
J’AI TOUJOURS EU LE MORAL, me sachant entre les mains d’une équipe compétente et à l’écoute. Durant mes séances de radiothérapie ou chimiothérapie, je ne me suis JAMAIS ARRÊTÉ DE TRAVAILLER afin de vivre normalement, comme tout le monde. Seul un arrêt de trois semaines m’a été imposé après la Curiethérapie, à cause d’une importante perte de poids et d’une grande fatigue.
J’ai présenté des réactions pendant la radiothérapie : la muqueuse buccale était rouge et sensible. Puis des réactions immédiates (dans les semaines qui ont suivi) transitoires juste après le traitement :
Une sécheresse buccale (hyposlalie) m’imposant de boire souvent
Une perte totale du goût associée à une langue noire (mycose), m’empêchant de faire une différence entre les aliments, suivie d’une perte de poids importante
La peau des joues sèche
Un double menton avec gonflement (« jabot » sous-mentonnier)
Des douleurs de langue liées à une ulcération en zone de curiethérapie Puis ces premières réactions sont rentrées dans l’ordre, tandis que je ne souffrais plus que de douleurs cervicales et aux oreilles, de temps en temps. J’ai tout de même perdu mon emploi… Alors j’ai décidé de créer mon entreprise, la maladie m’avait endurci…
Alors que je me croyais enfin tranquille, 8 ans après, j’ai souffert d’une forte douleur dentaire pour laquelle mon dentiste m’a extrait la dent de sagesse droite. Mais je n’ai pas cicatrisé, des douleurs locales puis dans le nerf dentaire sont apparues, malgré les extractions dentaires avant traitement de Radiothérapie et les gouttières fluorées après.
Une OSTEORADIONECROSE mandibulaire (trou osseux) s’est alors développée avec des douleurs mandibulaires violentes associées à une anesthésie du menton et des fourmillements sur le trajet du nerf dentaire inférieur, je me mordais la langue. Ces douleurs violentes ont été traitées par morphine de longs mois alors que je consultais le Centre antidouleur de l’Hôpital Bichât. Des traitements antibiotiques se sont succédés mais ne permettaient qu’un apaisement partiel et de quelques semaines. Après 6 mois d’évolution, Le Dr Delanian m’a proposé de bénéficier d’un traitement qu’elle avait mis au point pour cela (il n’avait pas encore de nom, mais depuis il s’appelle « PENTOCLO »). J’ai pris ce traitement pendant 3 ans, contrôlé par panoramique dentaire régulier. Petit à petit les douleurs ont diminué, le trou en bouche s’est refermé et les infections se sont faites plus rares… jusqu’à cicatrisation en surface d’abord dans la bouche, (un séquestre osseux ou morceau d’os « pourri » de 1 cm est sorti tout seul juste avant) ; puis en profondeur, permettant la réduction puis l’arrêt des fourmillements du menton en 3 ans. J’avoue avoir repris le tabac dans certains moments difficiles, ce qui n’aidait pas à contrôler la situation… Mais j’ai eu la joie de constater que je pouvais définitivement arrêter la morphine après quelques mois, puis que le niveau de l’os remontait sur les radios et le scanner mandibulaire. On a alors tout arrêté en 2007 ! C’était sans compter sur un nouvel événement… J’ai en effet souffert ensuite de mes artères du cou, la carotide gauche. J’ai présenté brutalement un épisode neurologique bref avec trouble de la vision, vertiges et troubles de la parole par accident vasculaire cérébral Ischémique. Le bilan a montré que j’avais une sténose serrée de l’artère carotide à 90 %, que l’on a pu rattacher aussi à la radiothérapie sans pour autant lui attribuer toute la faute puisque le tabac était passé par là…
La sténose carotide artérielle a été traitée par ENDARTERIECTOMIE et PONTAGE. Lors de cette intervention, le chirurgien a eu beaucoup de difficultés (qu’il a surmontées) en partie dues à la fibrose cervicale de la radiothérapie. J’ai pu à nouveau bénéficier du « PENTOCLO » 6 mois en post-opératoire pour aider à la récupération neurologique et la parole est revenue fluide à 95 %.
Ainsi, mon parcours médical sur 22 ans a été compliqué et douloureux, mais il a abouti à une guérison satisfaisante, non seulement du cancer mais des séquelles associées au traitement du cancer. Je tiens à remercier les médecins qui m’ont traité et soutenu pendant ces longues années, et qui m’accompagnent encore, en particulier le Dr Joël Depondt et le Dr Sylvie Delanian.

I testified during the French TV program « Allo Docteurs » on Monday May 23, 2016 on France 5, as a patient with latent after-effects of ENT (Ear Nose Throat) radiotherapy.
The office of our Association considered it useful that I set out, in an editorial, my journey that led me to a healing and an acceptable life. Therefore, 48 years old, I had ENT cancer in 1994. Unusual mouth odors had appeared: I consulted my usual dentist who, after examination, confirmed to me that the cause was not dental. A discomfort with swallowing was also present; my ear doctor noted an anomaly at the base of the tongue. After a biopsy, he told me that I had a cancerous tumor (an invasive undifferentiated ulcerating carcinoma).
So I was directed to a specialist ENT unit at Bichat Hospital (Dr Joël Depondt). At the end of this consultation, a surgical operation on the tongue seemed obvious despite the inevitable aesthetic consequences and the subsequent difficulties for speech and food. Needless to say, I was SHOCKED.
During another appointment with Dr Depondt, he informed me that at a meeting between doctors, including Dr Sylvie Delanian Oncologist Radiotherapist, he was offered treatment with EXCLUSIVE RADIOTHERAPY with Cobalt (at the dose of 45Gy) associated with CHEMOTHERAPY (Carboplatin) followed by CURITHERAPY with Iridium at a dose of 30 Gy (Dr Delanian Hôpital St-Louis).
Six weeks after this conservative treatment, the tumor had completely disappeared.
I HAVE ALWAYS BEEN UPBEAT, knowing that I am in the hands of a competent and attentive team. During my radiotherapy and chemotherapy treatment, I NEVER STOPPED WORKING, in order to live a normal life like everyone else. I was only forced to stop for three weeks after brachytherapy, due to significant weight loss and fatigue.
I experienced reactions during radiotherapy: the oral mucus membranes were red and tender. Then immediate transitory reactions (in the weeks that followed) just after treatment:
– Dry mouth (hyposalivation) requiring me to drink often
– Total loss of taste associated with a black tongue (yeast infection), preventing me from distinguishing between foods, followed by significant weight loss
– Dry cheek skin
– A double chin with swelling (« jabot » sub-chin)
– Tongue pains linked to an ulceration in the brachytherapy area.
Then these first reactions went away, and I only suffered from neck and ear pain from time to time. I still lost my job … So I decided to start my own business, the disease had hardened me …
When I finally thought all was well, 8 years later, I suffered from severe dental pain for which my dentist extracted the right wisdom tooth. But I did not heal; there was local pain and then pain in the dental nerve, despite having the dental extractions before radiotherapy treatment and the fluorinated aligners afterwards.
A mandibular OSTEORADIONECROSIS (hole in the bone) then developed with severe mandibular pain associated with anesthesia of the chin and tingling on the path of the lower dental nerve, causing me to bite my tongue. This severe pain was treated with morphine for many months while I was visiting the Pain Center at Bichât Hospital. Antibiotic treatments followed one another but only brought partial relief for a few weeks.
After 6 months of evolution, Dr Delanian offered me the possibility of benefiting from a treatment that she had developed for this (it did not yet have a name, but since then he has been called « PENTOCLO »). I took this treatment for 3 years, controlled by regular dental checkups. Little by little the pain diminished, the hole in the jawbone closed and infections became more rare… healing on the surface first in the mouth, (a bone sequestration or piece of “rotten” bone) 1 cm came out on its own just before; then healing in depth, allowing reduction and stopping of tingling in the chin in 3 years. I admit to having resumed smoking in certain difficult moments, which did not help the situation… But I was happy to note that I could definitively stop morphine after a few months, then that the level of bone went up on the radios and the mandibular scanner. We then stopped everything in 2007!
This was without counting on a new event … I indeed suffered then from my arteries of the neck, the left carotid. I was abruptly afflicted with a brief neurological case of vision disturbance, dizziness and speech impairment from an ischemic stroke. The assessment showed that I had a tight stenosis of the carotid artery at 90%, which we could also link to radiotherapy without attributing all the fault to it since the tobacco had passed there …
Carotid arterial stenosis was treated via an ENDARTERECTOMY and BRIDGE. During this intervention, the surgeon had many difficulties (which he overcame) partly due to the cervical fibrosis of the radiotherapy. I was again able to benefit from the « PENTOCLO » 6 months post-operatively to help neurological recovery and speech returned to 95% fluency.
Thus, my medical journey over 22 years was complicated and painful, but it resulted in a satisfactory cure, not only of cancer but of the after-effects associated with the treatment of cancer. I would like to thank the doctors who have treated and supported me for these long years, and who still accompany me, in particular Dr Joël Depondt and Dr Sylvie Delanian.
